EMPLOYMENT BENEFITS

Jennifer Hampton, LCSW is an independent contractor (not an employee) with GROW Counseling Network, LLC, which may be providing services through your employer (Insight Global, etc.). If you are being seen by referral via GROW Counseling Network, LLC, they will be coordinating payment of session fees through your employer directly, and you will not be responsible for payment unless you late-cancel or no-show for a session. Work obligations are not considered an extenuating circumstance that would waive my policy for missed sessions or late cancellations (i.e. within 72 hours of your scheduled appointment with me). Instead, you will be personally responsible to cover those session fees. Your employer should provide and protect time for your attendance in counseling/coaching sessions. Please discuss with HR or your leadership team how to effectively support time for your counseling sessions, and please advocate for yourself with your employer if scheduling conflicts arise.

PAYING THROUGH YOUR CLIENT PORTAL

The client portal I use for scheduling, charting and billing, SimplePractice, underwent one of the most extensive and strict payment security certification standards in the world—Payment Card Industry Data Security Standard (PCI DSS). This process requires merchants and service providers that store, process, or transmit customer-payment card data to adopt information security controls and processes to ensure data integrity. Credit card data is stored in the SimplePractice system and processed by Stripe—both of which are Level 1 PCI Data Security Standard Certified.

The system I use for appointments, billing and note-taking has several advantages, but it is not without some shortcomings. With COUPLE appointments, two clients will share billing. The client making the appointment will be considered the primary client. The primary client will be billed for the session and all billing documents will be in this client's name. There isn't a way to change the primary client for an appointment after it's created and saved.

For couple’s appointments, please select COUPLE from the drop-down menu and enter your partner, if allowed, as the second client. Only one member of the couple can receive an appointment reminder.

If you prefer to alternate payments between partners for sessions in the future or modify your payment schedule in some way, it is your responsibility to set up a way sort this out. Some partners choose to reimburse the primary client listed on the appointment. Others bring payment in the form of a check or cash at the time of the appointment so I can record who pays for the session.

INSURANCE & REIMBURSEMENT

At this time, Moxie LCSW Corp does not contract with any health insurance companies. Here is WHY. However, I will provide documentation to assist you with petitioning your health insurance company or employee assistance program to accept me on a "single case agreement" as an "out-of-network provider", and therefore reimbursing you for some or all of the cost of a certain number of therapeutic sessions.

If you plan to seek reimbursement from your insurance, health savings plan or Employee Assistance Program (EAP) after receiving services, please clarify with your insurance/employer the following before starting sessions:

‏    •    Benefits and scope of coverage
‏    •    Annual deductible
‏    •    Number of sessions allowed under coverage and process to appeal for more sessions
‏    •    Coverage amount per session
‏    •    Whether pre-authorization is required and whether referral is necessary

If you are hoping to get some money back for sessions, as allowed by certain insurance providers, funds and healthcare programs, I can provide a "superbill" to assist you. A superbill is an itemized form which details services provided to a client. It is the main data source for creation of a healthcare claim, which will be submitted by the client directly to their healthcare program for consideration for reimbursement of part or all of the session fee.

SLIDING SCALE

Deciding to embark on therapy may be one of the most important decisions you will make in your lifetime and one that should not force you to choose between basic needs and mental health treatment. Many people do not seek therapy because it seems unaffordable, or they abruptly discontinue therapy when their financial situation becomes unstable. Even those with health insurance are having problems accessing the mental health care they need.

Moxie LCSW Corp offers limited reduced fee services. If you're facing financial challenges that prohibit you from paying the standard rate per session (50 min), and you would like to be considered for a discounted rate, please let me know as soon as possible. I trust your assessment of your Personal Financial Experience to help guide this conversation. I also take into account extenuating circumstances that may further impact your financial experience (fires or natural disaster, pandemic, job loss, medical condition, separation, etc.).

Upon contracting for services with me, you will receive some information to protect you from excessive charges, which you can download for your records. Look for the document, Standard Notice and Consent Surprise Billing Protection Forms for Nonparticipating Providers.

Here are general guidelines about how I currently price my sliding scale to help you determine where you fall on it.

  • The highest dollar cost reflects the true cost of the service. It is the cost that the practitioner would charge all clients in the absence of a sliding scale. If you have access to financial security, rent high-end property, own property or have personal savings, you would not traditionally qualify for a reduced rate. If you are able to pay for “wants” and spend little time worried about securing necessities in your life, you can expect to be billed the full rate.

  • The reduced fee category at 10-15% off, reflects my acknowledgement that paying the full cost would prevent some folks from being able to participate, but who do not honestly find themselves reflected in either descriptions for the highest cost or the lowest.  If you are struggling to conquer debt or build savings or move away from paycheck-to-paycheck living, but have access to steady income and are not spending most of your time thinking about meeting basic needs such as food, shelter, medical care, child care, etc., a reduced fee might be most appropriate. If you, however, can ask others for financial support, such as family members, partners, or friends, please consider using those personal resources before you ask me about reducing my rate.

  • There are folks whose economic circumstances prevent them from accessing quality mental health treatment. If you struggle to afford health care, housing, food, child care, and are living paycheck-to-paycheck or are in significant debt, please reach out to me about GROW scholarships or referrals to community mental health agencies in your geographic area or online.  If you, however, can ask others for financial support, such as family members, partners, or friends, please consider using those personal resources before you ask me about a scholarship.

SCHOLOARSHIPS

Moxie LCSW Corp. partners with The GROW Restored Foundation Scholarship Awards program. Paraphrasing them... often, counseling is too expensive for people to obtain on their own, and we know even having health insurance doesn’t guarantee a person will receive the timely and good care they need. GROW Restored Foundation Scholarship Awards help assist the individual in supplementing what they can afford versus what therapy costs. Applications for a scholarship must be made directly by and at the discretion of the therapist and are approved by the foundation on a case-by-case basis. If a scholarship application is rejected, I will work with prospective clients to offer other solutions.

Please be mindful that if you expect a scholarship when you can truthfully afford the standard fees, you are limiting access to those who truly need the gift of financial flexibility.

Attached, you’ll find a graph illustrated with three bottles. Each bottle contains sentences that describe a person’s current financial experience and how that might align with my sliding scale.  At the end of the day, a sliding scale thrives on trust. Trust is a pretty amazing thing. I trust you to be honest in your assessment of your economic reality and let me know what fee is right for you, including whether you would like to be considered for a GROW scholarship (for individuals only).
